People on MS don't realize that modifiers exist for a reason. They are meant to fix problems like this when they arise.
To fix this problem, I would just create a role modifier.

Here are two you can use

Illegal (illegal alien)

- Cannot target or be targeted by Cops or Governors

I would use this on the doctor

Ascetic

- Use it 1x or 2x on the Cop

Follow The Cop isn't...inherently awful. It's okay if it shows up once in a while. But back in the old days, mafia roleblockers weren't in vogue just yet and the cop/doctor combination was outright expected in every single game. That led to weird situations where putting a cop but no doctor into the game was actually

for scum, because they'd keep shooting looking for a doctor they would never find.

Yeah, I've used that modifier in a game before. Just no specialized name for it.
