Some specific words/phrases move toward the "remove-immediately" arena because of memetic tendencies ("cocks", "BAS", "twatdonkey"... perhaps "douche", but I don't think I've removed it out-of-context).
There are a small number of words which are unacceptable in any context (other than, I suppose, a "What is acceptable?" discussion like this one). They should be obvious.
