If the owner of Alexander is killed in any way, the hammerer or killer dies and does not flip. (The owner of Alexander still dies, so there may be more to this).

If Alexander is used, it can target a person, killing them if they have any Geass powers.

No. Alexander is (unless something bastard is going on, as FA used magenta) not responsible for ThinkBig's death.

Alexander only operates when its owner dies. Culted did not die. The only way Alexander could possibly be responsible is if ThinkBig got it and somehow had the kill redirected to himself, thus killing himself with no flip. I will check whether even this is possible.
