Most of them are off-site. The modding team will create a forum like so: http://w11.zetaboards.com/Conclave/index/
Players will have accounts made for them, and given their own private subforum for confessionals, as well as each tribe will have a shared forum. If there are spectators, they two will have an area to discuss amongst each other. Right now, the example I linked, all subforums have been opened up so they're viewable to anyone.

However off-site games are mostly used for games that are anonymous. Some games are run here, but I'm less familiar with the specifics of those games as the last one I played in was years ago.

I ran a non-anon Survivor a year and a half or so ago. It's run pretty similarly to an anonymous game, but most of the communication happens through forum PMs, QuickTopics, or private chat like AIM or Skype. Spectators usually don't get anything special, but sometimes players make their own confessionals and the mod shares them with people. There's nothing wrong with either game type, but people tend to prefer anonymous games because they prevent meta-gaming and voting people out just because they know the player to be a threat, which happens in anonymous games.

Actually they aren't... I couldn't figure out how to give access to guests because as far as I can tell they should have it but they don't.

The flavor writers\challenge makers did a very thorough job.

I think it's mainly a difference of insane in a fun way and insane in an awful way. Although both were very entertaining for spectators, so who knows!
