We will all eventually die unless medical science gets really advanced and/or we all stop eating good foods like pizza and cheesecake.

Well, this is using your definition of "you". Not mine.

If you went into a machine which destroyed your current body but made an exact copy somewhere else out of different atoms, like teleporters in science fiction, would that still be you?

If it is, then you can't really say that "you return to your basic elements" since we just established that the idea of "you" doesn't depend on those specific particles.
