The loss of the thresher submarine and all crew, civilian government employees, contractors, and equipment onboard during routine sea trials due to condensation freezing a valve open led to the SUBSAFE programs.
The loss of multiple contractors and navy personnel onboard a nuclear aircraft carrier when they used the wrong material for a screw that then snapped when the primary-side steam system was lit up and filled with irradiated steam led to the quality assurance programs.
All these programs are good things.
None of those horrors are.
Being a dick to show a flaw in a system doesnt make you less of a dick. But thanks for making us realize the system was susceptible to dicks.
