If a message is in outbox but not sent, does that mean it wasn’t sent?

If it wasn’t sent? Why?

Outbox means the message was sent but wasn't opened. In messages with more than one recipient it moves to sent messages as soon as a single person opens the message

It does very rarely happen that a message leaves the outbox even if none of its recipients have read it, but I've never figured out why, or under what circumstances, that happens
