I'd prefer everyone start with the same amount of chips in new games. No one wants to go into a game with a disadvantage.

I'm fine with it at 16, making it longer will just make it take more time before I start doing stuff.

It'd be based off of the average of the other players. An example to better explain this:
5 players are currently playing.
Player 1 has $3750, Player 2 has $1500, Player 3 has $5000, Player 4 has $2750 and Player 5 has $3250. Player 6 joins in with $3250. Granted, this is a bit lower than most people, but it's the average amount compared to the other players.

Yes, this is intentional. The reason for this is because the people that have the blinds are not supposed to bet first (unless there are two-three players remaining, then it starts with the dealer on the pre-flop). After that, the person that is to the left of the dealer (next in the player list for forum games) starts the betting.

Hrm... this doesn't add up. Counting the pot and what everyone has, it totals to $4045, not $4000. I know I'm giving someone too much money somewhere.

I don't think the blinds have anything really to do with it, but maybe I am doing it because I already have money invested.

Found what my problem was: Hands 2 and 3 were calculated wrong. DiplomatDC's max total is actually $1695, not $1690. He started this hand with $1685 and has $1630 now; Orcinus_theoriginal has $780, not $785; Cheery Dog started this hand with $825 and has $770 now; MonkeyMan576 started this hand with $710 and has $700 now. Updating the OP for more accuracy purposes.
