Now, the first time I previewed, only the first spoiler was present, and it displayed the emoticon. This preview, no emoticon, just two colons and shifty. What gives?

You need to put a space before the colon to actually get the smiley. For some reason, using the preview function removes the space.
Clicked then immediately coded.(quotes added in after the fact because apparently ending spaces are even ignored in code tags)

So basically outside of spoiler tags, previewing removes the space that is added after the clicked emote, and in spoiler tags previewing removes both the space added before and after the emote, noting once again that emotes only work when there is a space before the colon. I don't know why any of this happens at all though.

tl;dr: If you have a smiley in your post, and then you preview it, doublecheck to make sure there is still a space before it, add one if necessary, and then don't preview again before posting.
